     今天有人问我：“高主任您好！请问美国做肩关节肩胛骨骨折经验最丰富的医生是谁？美国谁做肩关节肩胛骨骨折手术做的最多？”

     这是个很好的问题！

     美国的确有个做肩关节肩胛骨骨折最多的医生，经验也的确非常丰富。他就是肩胛骨骨折手术大师Peter A. Cole医生。

     Peter A. Cole医生1990年在美国迈阿密大学医学院毕业，接着在美国布朗大学做的实习医生，又在美国布朗大学做了骨科的住院医师，然后1998年在美国华盛顿西雅图harborview医学中心专门进修了创伤骨科。

     2002年Peter A. Cole医生应邀到美国明尼苏达大学 Saint Paul的Regions医院一级创伤中心创立一个骨科创伤分部。目前已由当初的3个骨科医生发展到了20多个骨科医生的团队。

     目前，Peter A. Cole医生担任美国明尼苏达大学Regions医院骨科主任。

     经过近30年在创伤骨科领域的摸爬滚打，Peter A. Cole医生在美国历史上积累了做肩胛骨骨折最多的病例记录。

    Peter A. Cole医生为什么会做了这么多的肩关节肩胛骨骨折手术？我觉得有其个人的努力和其优秀的平台有关。他所工作的美国一级创伤中心有大量的创伤患者。     好，我们一起复习一下美国创伤中心是怎么分级的。

    I级 该等级创伤中心为创伤患者提供最高等级的外科医疗，其年度接收的严重创伤患者也有一定的数量要求。它拥有的专家和仪器，覆盖各个专业，全天 24 h 待命。院内除了普通外科住院医师外，还要有外科医师、急诊医师、麻醉师、护师等，以及随时可获得的专家支持团队，能充分地响应各种类型的创伤需求和进行有效治疗，如骨科、神经外科、整形外科、麻醉学、急诊医学、放射学、内科医学、口腔颌面外科学、耳鼻喉科学、重症医学，以及康复服务。此外，该中心还有创伤研究和培训项目及创伤预防和随访项目。 I 级创伤中心还应是创伤教育和损伤预防方面的领军机构，同时是附近社区转诊的目的地。

    II级 该等级创伤中心常与丨级中心合作，提供综合的创伤医疗服务，有时会派人员补充到 I 级创伤机构，该中心配备有24h随时待命的专家、医务人员和仪器。在年度接收创伤患者的数量方面，可能取决于当地情况。对该级机构不做研究项目或外科住院医师项目的要求。

    III级 该级创伤中心没有全面的治疗各种类型创伤的专家，但是具有急救复苏、外科手术、重症监护等能治疗大部分创伤病人的资源。III级中心与I、II级中心有转诊协议，以及为乡村或社区医院特别严重的伤员提供资源支持。

    IV 级 /V 级 在美国一些州，由于没有资源建立III级创伤中心，因此IV级创伤中心应运而生。 IV级创伤中心提供并具有初步评估、稳定病情、诊断能力和向上一级转诊的能力，也提供一些手术和重症治疗服务，但需要依各个中心的创伤服务范围而定。人员配置方面，急诊室一般配备1名经过创伤专业培训的护士，而医师会在病人到达后及时出现。V 级与 IV 级在能力方面类似或稍差，但有可能非24h运营。

    在最高级别I级的美国创伤中心，再加上自己非常努力、好学、上进、热爱，造就了Peter A. Cole医生成了美国肩关节肩胛骨骨折手术病例最多的医生。

    Peter A. Cole医生不但手术做的好，还写了30余篇肩胛骨骨折相关的文章。今后我将分别分享里面的具体内容。希望他山之石可以攻玉，对我们中国人的肩关节肩胛骨骨折治疗也有参考价值。
