生物活性：克拉霉素具有广谱的抗菌活性。克拉霉素抑制 CYP3A4 催化的三唑仑 α-羟基化，其 IC₅₀ (K_i/ b>) 值为 56 (43) μM^[2]。克拉霉素显着抑制 HERG 钾电流^[3]。克拉霉素通过破坏连接 hERG1 和 PI3K 的信号通路影响自噬通量^{[ 4]}。   

体外：Clarithromycin 产生类似的浓度依赖性阻滞，IC₅₀ 为 45.7 µM^[3]。Clarithromycin 作用于所有细胞系，尤其是HCT116 细胞，24 小时后可诱导大量胞浆内空泡的形成。用 Clarithromycin（40、80 和 160 µM）长期治疗会改变细胞增殖并触发结直肠癌 (CRC) 细胞的凋亡细胞死亡。当克拉霉素重新加入细胞时，细胞增殖的抑制作用得到加强。特别是，在孵育 48 小时后重新加入 160 μM 克拉霉素，可在 72 小时时抑制细胞增殖。在 LS174T 细胞^[4] 中获得了类似的效果。克拉霉素（80 和 160 µM；48 小时）在剂量和时间上强烈增加 LC3-II/LC3-I 比率-依赖性方式，最大治疗时间为 24 小时。这种效应伴随着 p62/SQSTM1^[4] 的减少。

体内：200 mg/kg 的克拉霉素对体内测试的四种药物具有活性^[5]。
