    今天有人问我一个问题：“高主任您好！什么是不需要牵拉上臂的外旋法肩关节前脱位复位技术？”

    这是一个非常好的问题！

    外旋法肩关节前脱位复位技术最早是由Reinald Leidelmeyer在1977年描述的。2年后，Hennepin County Medical Center用这个方法进行了85例肩关节脱位的实践工作。1984年正式命名为：Hennepin手法。然后，2004年，Krishna Kiran Eachempati再此基础上又增加了外旋上臂之前前屈上臂20度这一动作。这一改良是为了胸大肌的力量和肩胛下肌的力量更加一致。所以现在采用外旋法复位肩关节前脱位时都加上上臂前屈20度这一动作。

    所以这一外旋复位技术又称为：1、Leidelmeye外旋复位法；2、Hennepin法；3、Eachempati屈曲-内收-外旋复位法。

    具体技术如下：

     患者躺在检查床上。医生站在床的一侧。患者上臂取内收位。医生一手抓住患者的腕关节，另一手稳定住患者的肘关节。将肘关节屈曲90度，将肩关节前屈20度，将上臂内收靠着胸壁。医生握着患者的手腕慢慢外旋患者的上臂。慢慢达到复位。注意：这个过程中并没有用力牵拉上臂。注意不要用太大的旋转力量以预防骨折。一般来说外旋到70到110度的时候肩关节就能够得以复位了。复位后将胳膊内旋到胸前。

    多个研究表明此方法的成功率在78%到100%之间。
